It may not be random rubbishness. The bowler may have meant it. I've tried it - fired it deliberately wide down the leg side, when a guy was charging - with a pre-arranged signal to the keeper that it was coming.
He 'forgot what the signal was for' and it went for four byes
Years ago when I was playing for Southwick Colts, I stumped someone off an off-side wide.
It's not all that uncommon.

A guess is the ball hit a helmet left behind the wicket keeper, OR tthey ran a tight single, a fielder had a shy at the stumps, missed and the ball went for 4 over-throws.
