Just got back from there mate. Had 7 days in Taormina and then 3 nights in Palermo. Main bit of advice would be use the train to get about rather than cars if you can, it's dirt cheap.
In Palermo, definitely definitely get the bus up to Monreale and spend a morning there. Apart from that, I wouldn't have said there were too many massive "must-see" sites, but just wandering the alleyways and seeing the little things (churches, markets, puppet museums) was great. Drinks are cheap which is bizarre for Italy. Our guidebook said that it was not really worth heading out in the evening but we found some really nice bars - a square full of really lively ones opposite the opera house.
If you do get a car, some of the small villages where they filmed bits of the Godfather are awesome.
We did the island in quite a lot of detail so PM me if you wanna know anything else, rather than me write an essay here!

Mt Etna, Taormina is nice with Roman amphitheatre but quite touristy.
