My brother is a surgeon, and he once had someone come in to casualty with a crossbow bolt through his head. This guy had tried to commit suicide by shooting a bolt through, from under his jaw, upwards. He fired, it went in and lodged coming out the top of his skull. He walked into casualty fully conscious - then as soon as someone tried to move the bolt a little, it must have ruptured a blood vessel and he died. Guess he got what he was after eventually.
