The same could be said of France at 2. A lot of it is down to competitive results and as there was a European tournament in the summer and the top 20 is generally dominated by Europe, at present the European teams are going to be holding the higher positions.

Yes the ranking is a points scheme thats continious.

Greece are about 20 now having gone up about 30 places due to Euro.
Englands win against Ukraine wont count for much being only a friendly against something like the 60th ranked team.
USA remember got as far as we did in the World Cup and have since had resepctable results against European teams.

Also they always do well in the Gold Cup, which often means then taking part in the Confederation Cup and they do sometimes get invited into the CONCAF thing and the Coppa America, so they play more competitive games and so move up the rankings.
