All down to the ref not being on top of the rules. Cannot sub during the shoot out except for if the goalkeeper is injured and only if you haven't used all your subs. Also, when the keeper was sent off, the opposition must nominate one of their players to be excluded from taking a penalty so both teams have the same number of penalty takers.

He did, but then immediately said he was told it wasn't for dissent, and was for coming off his line.
Ridiculously, for the save by the outfield player who went in goal, he was off his line.
The ref completely lost control.
