Shall wait and see if a ban follows seeing as Sky highlighted it.
There should be.

Ref probably thinks he saw it, but he clearly didn't. Definitely should be a ban, it was a violent premeditated spinning back elbow.

Just read the rules again, apparently IF the referee writes in his report that he saw a challenge/offence but didn't have a clear enough view to punish accordingly they can request a review.

only answer is for fourth official to have tv monitor away from touchline and, instead of having managers in his ear constantly, alert ref to any thing he may have missed ?

Or a 5th Official in the stands watching a monitor but that is too simple and would be immediate action. not retrospective so give Sky nothing to highlight and less to talk about.

He knew what he was doing and meant it. Sadly the championship doesn't have the same morals as the PL (Barton). So he will get away with it.
