What does a referee do when this happens?

Not sure but think if the ref is satisfied the ball would have entered the net then it's a goal. No different to the Liverpool beach ball incident which the ref didn't give but the concensus was that he should have done.

The ref did give the goal in regard to the beach ball fiasco.
That physio should be given a Knighthood and the fans should have had a whip-round to get the guy a brass(if they haven't already).

You cannot give a goal if the ball hasn't entered the net.
the only thing that can be done by the referee is to restart play with a drop ball and expel the physio from the game. He should not award a goal as play was stopped without the ball entering the goal.
