NSC's crazy 'Bomb Scare' bug - what's causing it?

Have people seen this bizarre bug that occasionally pops up? Users try to quote a post in a thread and instead this one randomly pops up...

I think it's a bug with multi-quote, it's happened to me a few times - but not recently.

Happen to me once multi quoting on tapatalk, even tried to edit the post but it still came up
