The elephant in the room is the refs are relaxed about the possibility of their making mistakes, very relaxed indeed, but the same does not apply to mistakes made by players. Two cards in five seconds is petulance by the ref.
However they are the referees so they must be in charge. That's fine...
My mate who lives in America watched the game and the media is saying:
"England star Lewis Dunk given ‘dumbest red card you can ever see’ after getting two yellow cards in 12 seconds"
But what do Americans know?

So it would surprise me not if the after-timing ref claims that he simply didn't think it necessary to show the second yellow when he red carded him.
Really. So a yellow for what he said and a red for carrying on? I only watched it once so apologies if I miss-saw.
Well I hope it was a straight red because we can then appeal.
He'd need to have done something different to be given a yellow for one and a red for another offence. You can't...
But he was not sent off for any of those offenses. He was given a second yellow. So whatever he did will be in a list of yellow card offenses, not red card offenses.
