I had it so it was surname first, then first name. - I guess i missed a line out when changing them around to make it easier to read :facepalm:

I had tried to use an image of the whole thing (done on excel) but couldn't find a way to get it to display and couldn't get a hosting site to work. (also a bit too wide)
