Why do we so often fall to pieces at season's end?

2012/13 - lose in playoff semi-finals
2014/15 - lose in playoff semi-finals
2015/16 - lose in playoff semi-finals
2016/17 - seven points ahead of Newcastle with three matches remaining and we fail to win any of them and have to settle for runners-up
2017/18 - one point from our last four games when we were in desperate need of points

Something's not right.

Thats a pretty good run to be an Albion fan, not a dull season in there. Careful what you wish for folks. Mid table obscurity is vastly over rated

13/14 - we had a great end-of-season run. Zero defeats in last 8 games, sneaked in to play-offs with last minute Ulloa goal at Forest. Not really good enough to go up, and lost in play-offs with God knows how many players out injured.
15/16 - amazing run-in - no defeats from mid-Feb to end of season. Incredibly unlucky to only draw at Boro in last game (Complete & Utter Shyster to blame). Lost in play-offs to Sheff Weds with four injuries in first leg, and about 30 chances on goal in second leg.
16/17 - it didn't matter!

I don't see it as a bad thing, in my opinion we've overperformed last couple of years to get where we are, and it's just not possible to do that for entire seasons at a time.

You are cherry picking. We didn't fall to pieces in any of those except the hangover after the celebrations last year. We were not favourites against Derby even before the injuries that left us playing with only one centre back twenty minutes into the second leg. We weren't favourites against Wednesday after the injuries of the first leg. We were favourites against Palace in 12/13, but it was close and we were up against a manager who had won the play offs a couple of years earlier. We were not expected to get anything at Everton, their home record has been their strength this season. We were the better side against Leicester, but got mugged. We mugged ourselves against Huddersfield and we started badly against Palace. These results are disappointing, but they are not a sign of falling apart. None of those results would be seen as shock defeats by the wider football world. We have to fight for everything we get in this division and, as Hughton says so often, success and failure is often a matter of fine margins. It is also not yet the end of the 2017/18 season. We still have 5 games to go. If you were to include the last nine games in all the other seasons, our record wouldn't be bad.

I'm beginning to get an idea - for the Hughton years at least. May well be rubbish, but I'll give it an airing anyway.
We all know CH isn't a manager who likes to chop and change (never been nicknamed 'The Tinkerman' has he!) and that's fine by me; he's done a great job - and I hope he carries on doing a great job for us. But...............yes, sorry, but.......maybe he needs to rotate just a little bit more.
This season we haven't had any choice but to play Stephens and Propper in midfield - and we were damn lucky to get away with no cover - but they're both shagged out now. It's lucky that Kayal has got fit just in time, but even so, I'm pretty sure if Stephens hadn't been injured and Propper suspended Kayal wouldn't have got a look in. But, we did have the cover at CB to give Connor (or Uwe) a bit of game time, or even a start or two. We didn't, and now with five games to go, the Dunk'n'Duffy pairing is running on empty too.
In 2016, when we hit suspensions and injuries at the end of the season, the replacements who came in for the play offs weren't up to speed due to lack of game time. Last year we probably just ran out of steam as much as going on holiday on the beach - and didn't we bring in someone not match ready to have a nightmare in that last match at Villa - Rosenior, was it?

Maybe - just maybe - if we had rotated our CBs just a little bit, occasionally, it would be easier to 'rest' Duffy for a couple of games now rather than have issues with 'dropping' him.

Good to see someone composing a sensible response to my post. But I can't help believing that we have a recent tendency to run out of steam at the end of the season.
